Gre za izjemno zmogljivo orodje, ki pa se v vsakdanji diagnostiki doma\u010dih \u017eivali, predvsem psov in ma\u010dk, \u0161e vedno razmeroma redko uporablja.<\/p>\n<p>Zaradi neprimerljive visoke lo\u010dljivosti in zmo\u017enosti prikaza ultrastrukturnih podrobnosti nam elektronska mikroskopija omogo\u010da pridobivanje klju\u010dnih informacij o \u0161tevilnih patolo\u0161kih procesih, od virusnih oku\u017eb do ledvi\u010dnih bolezni in neoplasti\u010dnih sprememb.<\/p>\n<p>V tem \u010dlanku predstavljamo klju\u010dna podro\u010dja uporabe elektronske mikroskopije v veterinarski diagnostiki pri psih in ma\u010dkah, njene prednosti, omejitve in izzive ter prihodnje razvojne usmeritve.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Elektronski mikroskop v laboratoriju MyLav: ena na\u0161ih najpomembnej\u0161ih pridobitev!<\/strong><\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter size-full wp-image-32233\" src=\"https:\/\/www.mylav.net\/wp-content\/uploads\/2025\/07\/fra_01.jpg\" alt=\"\" width=\"454\" height=\"255\" \/><\/p>\n<p>&nbsp;<\/p>\n<p><strong>Kaj je elektronska mikroskopija?<\/strong><\/p>\n<p>Elektronska mikroskopija je tehnika, ki namesto svetlobe za osvetlitev vzorca tkiva uporablja snop elektronov. S tem je mogo\u010de dose\u010di bistveno vi\u0161jo lo\u010dljivost kot pri klasi\u010dnem svetlobnem mikroskopu, vse do lo\u010dljivosti pod enim nanometrom.<\/p>\n<p>Obstajata dve osnovni vrsti elektronske mikroskopije:<\/p>\n<ul>\n<li><strong>Presevna (transmisijska) elektronska mikroskopija (TEM)<\/strong>: elektroni prehajajo skozi izjemno tanke sloje tkiva in pri tem ustvarijo dvodimenzionalne slike z visoko lo\u010dljivostjo. Idealna je za opazovanje znotraj celi\u010dnih struktur.<\/li>\n<li><strong>Vrsti\u010dna elektronska mikroskopija (SEM)<\/strong>: snop elektronov \u00bbskenira\u00ab povr\u0161ino in ustvari tridimenzionalne slike. Uporabna je za preu\u010devanje oblike in strukture tkivnih povr\u0161in. Danes se elektronska mikroskopija uporablja predvsem v raziskovalne namene.<\/li>\n<\/ul>\n<p>V veterinarski diagnostiki se najpogosteje uporablja presevna elektronska mikroskopija (TEM), saj omogo\u010da vpogled v strukturne podrobnosti, ki so klju\u010dne za patolo\u0161ko in virolo\u0161ko diagnostiko.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Uporaba v diagnostiki pri psih in ma\u010dkah<\/strong><\/p>\n<ol>\n<li><strong>Virusne oku\u017ebe<\/strong><\/li>\n<\/ol>\n<p>TEM igra klju\u010dno vlogo pri neposrednem dolo\u010danju virusov v tkivih, \u0161e posebej takrat, ko obi\u010dajni testi (PCR ali imunohistokemija) ne dajo jasnega odgovora ali niso na voljo.\u00a0Na primer:<br \/>\n\u2013\u00a0<strong>Virus pasje kuge (CDV)<\/strong>: prisotnost vklju\u010dkov v nevronih ali epitelnih celicah lahko potrdimo s TEM, pri \u010demer prepoznamo zna\u010dilne delce virusa.<br \/>\n\u2013\u00a0<strong>Ma\u010dji infekcijski peritonitis (FIP)<\/strong>: koronavirus, ki ga povzro\u010da, lahko opazujemo znotraj makrofagov, kar pomaga razlikovati to stanje od drugih granulomatoznih bolezni.<\/p>\n<ol>\n<li><strong>Ledvi\u010dne bolezni<\/strong><\/li>\n<\/ol>\n<p>Za dokon\u010dno dolo\u010ditev tipa glomerularne bolezni pri psih in ma\u010dkah je pogosto potrebna ultrastrukturna analiza. \u010ceprav svetlobna mikroskopija in imunofluorescenca nudita uporabne informacije, je TEM nepogre\u0161ljiva za:<br \/>\n\u2013 odkrivanje <strong>zlepljenih prstastih podalj\u0161kov podocitov<\/strong>, kar je zna\u010dilno pri glomerularni bolezni z minimalnimi lezijami.<br \/>\n\u2013 prepoznavanje <strong>kopi\u010denja imunskih kompleksov v <\/strong>glomerulih pri imunsko odvisnih glomerularnih boleznih.<br \/>\n\u2013 opredelitev <strong>sprememb bazalne membrane<\/strong>, ki se lahko pojavijo pri dednih nefropatijah.<\/p>\n<ol>\n<li><strong>Ciliarna diskinezija<\/strong><\/li>\n<\/ol>\n<p>Primarna ciliarna diskinezija je prirojena bolezen, pri kateri je motena gibljivost migetalk, kar vodi v kroni\u010dne oku\u017ebe dihal. TEM omogo\u010da neposredno opazovanje nepravilnosti, najpogosteje gre za manjkajo\u010de dineinske ro\u010dice (robne ali sredi\u0161\u010dne) ali nepravilno ureditev mikrotubulov v migetalki. TEM predstavlja zlati diagnosti\u010dni standard za preiskavo te bolezni pri psih.<\/p>\n<ol>\n<li><strong>Bolezni kopi\u010denja in lizosomske patolo\u0161ke spremembe<\/strong><\/li>\n<\/ol>\n<p>Nekatere presnovne bolezni, kot so <strong>gangliozidoza<\/strong>, <strong>mukopolisaharidozae<\/strong> ali<strong> nevronska ceroidna lipofuscinoza<\/strong>, ka\u017eejo zna\u010dilne citoplazemske vklju\u010dke, ki jih zanesljivo prepoznamo prav s presevno elektronsko mikroskopijo. Opazovanje zebrastih telesc ali membranskih\/citoplazemskih vklju\u010dkov pogosto potrdi sum na nevrodegenerativno bolezen ali sistemsko motnjo kopi\u010denja.<\/p>\n<ol>\n<li><strong>Neoplazije in karakterizacija tumorjev<\/strong><\/li>\n<\/ol>\n<p>\u010ceprav ostajata histopatologija in imunohistokemija glavni steber diagnostike rakavih obolenj, pride TEM prav ravno v dvoumnih primerih, kjer potrebujemo dodatno ultrastrukturno potrditev. S TEM je mogo\u010de:<br \/>\n\u2013 razlikovati med karcinomi, sarkomi ali slabo diferenciranimi okrogloceli\u010dnimi tumorji na podlagi strukture organelov, sekretornih granul ali vzorca intermediarnih filamentov.<br \/>\n\u2013 oceniti prisotnost azbestnih vlaken pri pri mezoteliomih ali drugih novotvorbah.<\/p>\n<ol>\n<li><strong>Zahtevne ko\u017ene bolezni<\/strong><\/li>\n<\/ol>\n<p>Pri bolezni, kot so <strong>lupus<\/strong>, <strong>bulozni pemfigoid<\/strong> ali <strong>druge avtoimunske bolezni, ki povzro\u010dajo mehurje na ko\u017ei,<\/strong> lahko s TEM prepoznamo:<br \/>\n\u2013 spremembe do globine dermoepidermalne meje;<br \/>\n\u2013 prisotnost elektronsko gostih depozitov vzdol\u017e bazalne membrane;<br \/>\n\u2013 ultrastrukturne spremembe keratinocitov.<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Prednosti elektronske mikroskopije<\/strong><\/p>\n<p>&nbsp;<\/p>\n<ol>\n<li><strong>Visoka lo\u010dljivost<br \/>\n<\/strong>TEM omogo\u010da prepoznavanje celi\u010dnih in subceli\u010dnih struktur, kot so mitohondriji, endoplazemski retikulum in jedrske pore, ki so pod svetlobnim mikroskopom popolnoma nevidne. Zato je nepogre\u0161ljiva v primerih, ko so za diagnozo odlo\u010dilne \u017ee najmanj\u0161e ultrastrukturne spremembe.<\/li>\n<li><strong>Potrditvena in dopolnilna vloga<br \/>\n<\/strong>TEM deluje kot potrditveno orodje, kadar histologija in imunohistokemija dajeta nejasne rezultate, s \u010dimer pogosto dopolnjuje diagnosti\u010dni postopek.<\/li>\n<li><strong>Arhiviranje in ponovna uporaba<br \/>\n<\/strong>Analizirane vzorce je mogo\u010de shraniti in ponovno uporabiti kot referenco za prihodnje raziskovalne \u0161tudije ali retrospektivne primerjave.<\/li>\n<\/ol>\n<p><strong>\u00a0<\/strong><\/p>\n<p><strong>Omejitve in izzivi<\/strong><\/p>\n<p>Kljub svoji uporabnosti se presevna elektronska mikroskopija (TEM) v rutinski diagnostiki hi\u0161nih \u017eivali uporablja razmeroma redko. Glavne omejitve so:<\/p>\n<p>&nbsp;<\/p>\n<ol>\n<li><strong>Stro\u0161ki in dostopnost<br \/>\n<\/strong>Oprema je draga, za delo z njo pa je potreben visoko usposobljen strokovni kader. Na voljo je le v malo\u0161tevilnih raziskovalnih sredi\u0161\u010dih in laboratorijih.<\/li>\n<li><strong><br \/>\nPriprava vzorcev<br \/>\n<\/strong>Priprava je bistveno bolj zapletena kot pri standardni histopatologiji. Tkiva je treba fiksirati v glutaraldehidu, vklju\u010diti v smolo, narezati na izjemno tanke preseke in obarvati s te\u017ekimi kovinami (uranilacetat in svin\u010dev citrat).<\/li>\n<\/ol>\n<p>&nbsp;<\/p>\n<ol start=\"3\">\n<li><strong> Zahtevna interpretacija<br \/>\n<\/strong>Interpretacija slik zahteva visoko raven strokovnega znanja. \u010ce patolog nima ustrezne strokovne usposobljenosti in izobrazbe, lahko pride do napak.<\/li>\n<\/ol>\n<p>&nbsp;<\/p>\n<p>V zadnjih letih smo se v <strong>laboratoriju MyLav<\/strong> opremili s presevnim elektronskim mikroskopom, kar nam omogo\u010da rutinsko izvajanje ultrastrukturnih preiskav na razli\u010dnih diagnosti\u010dnih podro\u010djih.
