{"id":48512,"date":"2025-12-27T19:38:30","date_gmt":"2025-12-27T18:38:30","guid":{"rendered":"https:\/\/www.mylav.net\/?p=48512"},"modified":"2026-01-06T19:55:25","modified_gmt":"2026-01-06T18:55:25","slug":"alterations-morphologiques-des-leucocytes-et-autres-cellules-nucleees-du-sang-peripherique","status":"publish","type":"post","link":"https:\/\/www.mylav.net\/fr\/hematologie\/alterations-morphologiques-des-leucocytes-et-autres-cellules-nucleees-du-sang-peripherique\/","title":{"rendered":"ALT\u00c9RATIONS MORPHOLOGIQUES DES LEUCOCYTES ET AUTRES CELLULES NUCL\u00c9\u00c9ES DU SANG P\u00c9RIPH\u00c9RIQUE"},"content":{"rendered":"<p>Dans cet article, nous abordons un aspect fondamental de l\u2019analyse du frottis sanguin : l\u2019identification des <strong>caract\u00e9ristiques morphologiques<\/strong> des principaux leucocytes (granulocytes neutrophiles, monocytes, lymphocytes), ainsi que la mise en \u00e9vidence d\u2019<strong>autres cellules nucl\u00e9\u00e9es<\/strong> circulantes, notamment des cellules non classifiables, des \u00e9rythrocytes nucl\u00e9\u00e9s et des mastocytes.<\/p>\n<p><strong>GRANULOCYTES NEUTROPHILES EN BANDE &#8211; \u00ab LEFT SHIFT \u00bb<\/strong><\/p>\n<p>On parle de <strong>\u00ab left shift \u00bb<\/strong> ou <strong>d\u00e9viation \u00e0 gauche<\/strong> lorsqu\u2019on observe dans le sang une augmentation de granulocytes neutrophiles non segment\u00e9s, donc immatures.<br \/>\nLes formes les plus fr\u00e9quemment rencontr\u00e9es sont les <strong>neutrophiles immatures <\/strong>(band cells). En pr\u00e9sence de stimuli inflammatoires s\u00e9v\u00e8res, on peut \u00e9galement observer des stades de maturation plus pr\u00e9coces (ex. m\u00e9tamy\u00e9locytes, my\u00e9locytes).<\/p>\n<p>On distingue classiquement :<\/p>\n<ul>\n<li><strong>Left shift r\u00e9g\u00e9n\u00e9ratif<\/strong> :<br \/>\nle nombre de neutrophiles immatures est <strong>inf\u00e9rieur<\/strong> \u00e0 celui des neutrophiles segment\u00e9s, et l\u2019on observe le plus souvent une <strong>neutrophilie<\/strong>.<\/li>\n<li><strong>Left shift d\u00e9g\u00e9n\u00e9ratif<\/strong> :<br \/>\nle nombre de neutrophiles immatures est <strong>sup\u00e9rieur<\/strong> \u00e0 celui des segment\u00e9s ; dans ce cas, on met plus volontiers en \u00e9vidence une <strong>neutrop\u00e9nie<\/strong> ou un nombre de neutrophiles dans l\u2019intervalle de r\u00e9f\u00e9rence.<br \/>\nCe profil h\u00e9matologique est g\u00e9n\u00e9ralement associ\u00e9 \u00e0 des <strong>processus inflammatoires plus graves<\/strong> et \u00e0 un <strong>pronostic d\u00e9favorable<\/strong>.<\/li>\n<\/ul>\n<figure id=\"attachment_48513\" aria-describedby=\"caption-attachment-48513\" style=\"width: 768px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" class=\"size-medium_large wp-image-48513\" src=\"https:\/\/www.mylav.net\/wp-content\/uploads\/2026\/01\/bf01-768x528.jpg\" alt=\"\" width=\"768\" height=\"528\" \/><figcaption id=\"caption-attachment-48513\" class=\"wp-caption-text\">Figure 1. Sang p\u00e9riph\u00e9rique, chat (40x) : pr\u00e9sence de granulocytes neutrophiles immatures.<\/figcaption><\/figure>\n<p>&nbsp;<\/p>\n<p>La pr\u00e9sence <strong>exclusive<\/strong> de neutrophiles hyposegment\u00e9s sur un frottis sanguin peut \u00eatre observ\u00e9e lors d\u2019<strong>anomalie de Pelger-Hu\u00ebt<\/strong>, une affection h\u00e9r\u00e9ditaire rare caract\u00e9ris\u00e9e par un d\u00e9faut de maturation nucl\u00e9aire entra\u00eenant une <strong>hyposegmentation<\/strong> des neutrophiles, mais aussi des \u00e9osinophiles et basophiles.<\/p>\n<p>Cette anomalie est surtout d\u00e9crite chez le chien, en particulier dans les races :<br \/>\n<strong>Berger Australien, Samoy\u00e8de, Basenji, Cocker Spaniel, Border Collie, Foxhound, Bouvier Australien, Boston Terrier, Berger Allemand<\/strong>.<\/p>\n<p>Les animaux <strong>h\u00e9t\u00e9rozygotes<\/strong> pour cette anomalie sont asymptomatiques (sans augmentation de la susceptibilit\u00e9 aux infections), car les neutrophiles ne pr\u00e9sentent pas de <strong>d\u00e9faut fonctionnel<\/strong>. La d\u00e9couverte est le plus souvent <strong>fortuite<\/strong> et d\u00e9pourvue de signification clinique.<\/p>\n<p>Il existe \u00e9galement des formes <strong>acquises<\/strong> (pseudo Pelger-Hu\u00ebt) dans lesquelles la majorit\u00e9 des granulocytes appara\u00eet hyposegment\u00e9e, en lien avec une <strong>syndrome my\u00e9lodysplasique<\/strong>, l\u2019administration de certains m\u00e9dicaments modifiant la granulopo\u00ef\u00e8se, ou au cours de <strong>left shift d\u00e9g\u00e9n\u00e9ratifs s\u00e9v\u00e8res<\/strong>.<\/p>\n<figure id=\"attachment_48522\" aria-describedby=\"caption-attachment-48522\" style=\"width: 768px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" class=\"size-medium_large wp-image-48522\" src=\"https:\/\/www.mylav.net\/wp-content\/uploads\/2026\/01\/bf02-768x286.jpg\" alt=\"\" width=\"768\" height=\"286\" \/><figcaption id=\"caption-attachment-48522\" class=\"wp-caption-text\">Figure 2. Sang p\u00e9riph\u00e9rique, chien Berger Australien (40x) : neutrophiles pr\u00e9sentant une hyposegmentation nucl\u00e9aire (anomalie de Pelger-Hu\u00ebt).<\/figcaption><\/figure>\n<p>&nbsp;<\/p>\n<p><strong>GRANULOCYTES NEUTROPHILES \u00c0 HYPERSEGMENTATION NUCL\u00c9AIRE &#8211; \u00ab RIGHT SHIFT \u00bb<\/strong><\/p>\n<p>On parle de <strong>\u00ab right shift \u00bb<\/strong> ou <strong>d\u00e9viation \u00e0 droite<\/strong> lorsqu\u2019on observe une <strong>pr\u00e9dominance nette<\/strong> de neutrophiles <strong>hypersegment\u00e9s<\/strong> (plus de 5 lobes nucl\u00e9aires chez le chien et le chat).<\/p>\n<p>Ces alt\u00e9rations sont li\u00e9es \u00e0 une <strong>dur\u00e9e de s\u00e9jour prolong\u00e9e en circulation<\/strong>, conduisant \u00e0 un \u00ab vieillissement \u00bb des neutrophiles dans le sang p\u00e9riph\u00e9rique. Elles peuvent \u00eatre observ\u00e9es en cas de :<\/p>\n<ul>\n<li>traitement par <strong>glucocortico\u00efdes<\/strong>,<\/li>\n<li><strong>hyperadr\u00e9nocorticisme<\/strong>,<\/li>\n<li>maladies inflammatoires chroniques,<\/li>\n<li>processus my\u00e9lodysplasiques,<\/li>\n<li>leuc\u00e9mies my\u00e9lo\u00efdes.<\/li>\n<\/ul>\n<figure id=\"attachment_48531\" aria-describedby=\"caption-attachment-48531\" style=\"width: 768px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" class=\"size-medium_large wp-image-48531\" src=\"https:\/\/www.mylav.net\/wp-content\/uploads\/2026\/01\/bf03-768x392.jpg\" alt=\"\" width=\"768\" height=\"392\" \/><figcaption id=\"caption-attachment-48531\" class=\"wp-caption-text\">Figure 3. Sang p\u00e9riph\u00e9rique, cheval (40x) : hypersegmentation des neutrophiles dans le cadre d\u2019une atteinte h\u00e9patique.<\/figcaption><\/figure>\n<p>&nbsp;<\/p>\n<p><strong>GRANULOCYTES NEUTROPHILES TOXIQUES<\/strong><\/p>\n<p>Le terme de <strong>\u00ab toxicit\u00e9 \u00bb neutrophilique<\/strong>, emprunt\u00e9 \u00e0 la m\u00e9decine humaine (d\u00e9crit initialement chez des patients pr\u00e9sentant endotox\u00e9mie et sepsis \u00e0 bact\u00e9ries Gram n\u00e9gatif), d\u00e9signe la pr\u00e9sence d\u2019<strong>anomalies microscopiques<\/strong> des neutrophiles circulants, cons\u00e9cutives \u00e0 des <strong>alt\u00e9rations structurales<\/strong> survenant au niveau de la moelle osseuse (ou d\u2019autres sites d\u2019h\u00e9matopo\u00ef\u00e8se extra-m\u00e9dullaire) avant leur lib\u00e9ration dans le sang.<\/p>\n<p>Ces d\u00e9fauts de maturation sont rattach\u00e9s \u00e0 une <strong>granulopo\u00ef\u00e8se acc\u00e9l\u00e9r\u00e9e<\/strong>, c\u2019est-\u00e0-dire \u00e0 une production accrue associ\u00e9e \u00e0 une <strong>r\u00e9duction du temps de maturation<\/strong>, en r\u00e9ponse \u00e0 une forte demande de l\u2019organisme.<\/p>\n<p>Les diff\u00e9rents <strong>signes de toxicit\u00e9<\/strong> observables sur un <strong>frottis sanguin color\u00e9 au Romanowsky<\/strong> concernent principalement le cytoplasme, et peuvent \u00eatre d\u00e9crits, par ordre <strong>croissant de gravit\u00e9<\/strong> :<\/p>\n<p>&nbsp;<\/p>\n<p><strong>BASOPHILIE CYTOPLASMIQUE<\/strong><\/p>\n<p>Coloration plus bleu-azur\u00e9e du cytoplasme, li\u00e9e \u00e0 la <strong>persistance de r\u00e9ticulum endoplasmique rugueux (RER) et de polyribosomes<\/strong>, qui ne devraient plus \u00eatre pr\u00e9sents dans les neutrophiles matures.<\/p>\n<p>Si cette modification est observ\u00e9e <strong>uniquement chez les neutrophiles immatures<\/strong>, et non chez les segment\u00e9s, elle ne doit pas \u00eatre interpr\u00e9t\u00e9e comme un signe de toxicit\u00e9, mais plut\u00f4t comme un reflet de <strong>l\u2019immaturit\u00e9<\/strong> de la cellule.<\/p>\n<figure id=\"attachment_48540\" aria-describedby=\"caption-attachment-48540\" style=\"width: 768px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" class=\"size-medium_large wp-image-48540\" src=\"https:\/\/www.mylav.net\/wp-content\/uploads\/2026\/01\/bf04-768x564.jpg\" alt=\"\" width=\"768\" height=\"564\" \/><figcaption id=\"caption-attachment-48540\" class=\"wp-caption-text\">Figure 4. Sang p\u00e9riph\u00e9rique, chien (100x) : neutrophiles avec cytoplasme d\u2019aspect mousseux.<\/figcaption><\/figure>\n<p>&nbsp;<\/p>\n<p><strong>ASYNCHRONIE MATURATIVE &#8211; SIGNES DE DYSPLASIE<\/strong><\/p>\n<p>Pr\u00e9sence d\u2019<strong>anomalies asynchrones<\/strong> au sein de la m\u00eame cellule, refl\u00e9tant des <strong>troubles de la granulopo\u00ef\u00e8se<\/strong>.<br \/>\nExemples :<\/p>\n<ul>\n<li>cytoplasme paraissant mature avec une <strong>chromatine nucl\u00e9aire encore immature<\/strong>,<\/li>\n<li>noyaux en anneau,<\/li>\n<li>neutrophiles g\u00e9ants lib\u00e9r\u00e9s trop pr\u00e9cocement par la moelle (saut d\u2019une division cellulaire), et donc de <strong>taille sup\u00e9rieure \u00e0 un neutrophile normal<\/strong>.<\/li>\n<\/ul>\n<p>Ces cellules g\u00e9antes sont plus fr\u00e9quemment observ\u00e9es chez le chat, d\u00e9crites notamment dans le cadre de <strong>dyscrasie du Caniche<\/strong>, d\u2019infection par le <strong>FeLV<\/strong> et lors de traitements <strong>chimioth\u00e9rapeutiques<\/strong>.<\/p>\n<figure id=\"attachment_48567\" aria-describedby=\"caption-attachment-48567\" style=\"width: 768px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" class=\"size-medium_large wp-image-48567\" src=\"https:\/\/www.mylav.net\/wp-content\/uploads\/2026\/01\/bf07-768x326.jpg\" alt=\"\" width=\"768\" height=\"326\" \/><figcaption id=\"caption-attachment-48567\" class=\"wp-caption-text\">Figure 7. Sang p\u00e9riph\u00e9rique, chat (100x). \u00c0 gauche : gigantisme cellulaire (fl\u00e8che rouge). \u00c0 droite : noyau en anneau.<\/figcaption><\/figure>\n<p>&nbsp;<\/p>\n<p><strong>GRANULES PRIMAIRES<\/strong><\/p>\n<p>Persistance, chez des neutrophiles matures ou immatures, de <strong>fines granulations magenta<\/strong> normalement visibles uniquement au stade promy\u00e9locyte.<br \/>\nIl s\u2019agit d\u2019une <strong>d\u00e9couverte tr\u00e8s rare<\/strong> chez les animaux domestiques.<\/p>\n<p>Ces alt\u00e9rations h\u00e9matologiques sont le plus souvent associ\u00e9es \u00e0 des <strong>inflammations syst\u00e9miques<\/strong> (et plus rarement localis\u00e9es), le plus souvent d\u2019origine \u00a0<strong>bact\u00e9rienne<\/strong>, mais pas exclusivement. Elles sont \u00e9galement d\u00e9crites en cas de :<\/p>\n<ul>\n<li>maladies virales (parvovirose, virus respiratoires f\u00e9lins),<\/li>\n<li>an\u00e9mie h\u00e9molytique \u00e0 m\u00e9diation immune,<\/li>\n<li>coagulation intravasculaire diss\u00e9min\u00e9e chez le chien,<\/li>\n<li>certains troubles m\u00e9taboliques (insuffisance r\u00e9nale aigu\u00eb, acidoc\u00e9tose diab\u00e9tique, lipidose &#8211; particuli\u00e8rement chez le chat).<\/li>\n<\/ul>\n<p>Dans ces contextes, les signes de toxicit\u00e9 sont g\u00e9n\u00e9ralement <strong>moins marqu\u00e9s<\/strong> que lors d\u2019un <strong>processus septique grave et aigu<\/strong>.<\/p>\n<p>Chez le chien, des signes de toxicit\u00e9 neutrophilique s\u00e9v\u00e8re, surtout associ\u00e9s \u00e0 une neutrop\u00e9nie, sont corr\u00e9l\u00e9s \u00e0 une <strong>dur\u00e9e d\u2019hospitalisation plus longue<\/strong> et \u00e0 une <strong>mortalit\u00e9 plus \u00e9lev\u00e9e<\/strong>. \u00c0 l\u2019inverse, la r\u00e9solution de la toxicit\u00e9 apr\u00e8s traitement est interpr\u00e9t\u00e9e comme un <strong>facteur pronostique favorable<\/strong>.<\/p>\n<p>Chez le chat, les signes de toxicit\u00e9 sont \u00e9galement associ\u00e9s \u00e0 un <strong>temps d\u2019hospitalisation plus long<\/strong>, mais <strong>sans valeur pronostique n\u00e9gative d\u00e9montr\u00e9e<\/strong>.<\/p>\n<p>Enfin, il est important de rappeler que certaines <strong>alt\u00e9rations cytoplasmiques<\/strong> des neutrophiles peuvent \u00eatre <strong>art\u00e9factuelles<\/strong>, en cas de d\u00e9lai prolong\u00e9 entre le pr\u00e9l\u00e8vement et la pr\u00e9paration du frottis, notamment lors de contact prolong\u00e9 avec l\u2019EDTA : on peut observer des <strong>ponctuations basophiles<\/strong> et des <strong>vacuolisations \u00e0 contours nets<\/strong>, pouvant mimer les signes de toxicit\u00e9 d\u00e9crits ci-dessus.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>MONOCYTES ACTIV\u00c9S<\/strong><\/p>\n<p>Il s\u2019agit de monocytes dont le cytoplasme appara\u00eet <strong>plus basophile<\/strong> ou d\u2019<strong>aspect plus mousseux<\/strong>, souvent en lien avec une <strong>monocytose<\/strong>. Leur pr\u00e9sence peut \u00eatre associ\u00e9e \u00e0 des processus inflammatoires aigus ou chroniques, infectieux (ex. bact\u00e9ries du genre <em>Anaplasma spp<\/em>, <em>Ehrlichia canis<\/em>, protozoaires tels que <em>Babesia spp<\/em> et <em>Leishmania spp<\/em>, infections fongiques) ou non infectieux (ex. maladies \u00e0 m\u00e9diation immune, n\u00e9crose).<\/p>\n<p>Des alt\u00e9rations morphologiques marqu\u00e9es des monocytes, leur conf\u00e9rant un aspect <strong>\u00ab macrophagique \u00bb<\/strong>, sont d\u00e9crites dans des contextes :<\/p>\n<ul>\n<li>d\u2019inflammation s\u00e9v\u00e8re,<\/li>\n<li>de n\u00e9crose,<\/li>\n<li>de n\u00e9oplasies d\u2019origine histiocytaire ou d\u2019autres types (ex. lymphome).<\/li>\n<\/ul>\n<p>Les patients pr\u00e9sentant ces caract\u00e9ristiques h\u00e9matologiques ont tendance \u00e0 avoir un <strong>pronostic plus d\u00e9favorable<\/strong>, en particulier lorsqu\u2019une n\u00e9oplasie est associ\u00e9e.<\/p>\n<figure id=\"attachment_48576\" aria-describedby=\"caption-attachment-48576\" style=\"width: 768px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" class=\"size-medium_large wp-image-48576\" src=\"https:\/\/www.mylav.net\/wp-content\/uploads\/2026\/01\/bf08-768x274.jpg\" alt=\"\" width=\"768\" height=\"274\" \/><figcaption id=\"caption-attachment-48576\" class=\"wp-caption-text\">Figure 8. Pr\u00e9sence de monocytes activ\u00e9s dans un contexte d\u2019infection \u00e0 Babesia canis.<\/figcaption><\/figure>\n<p>&nbsp;<\/p>\n<p><strong>LYMPHOCYTES ACTIV\u00c9S<\/strong><\/p>\n<p>Il s\u2019agit le plus souvent de <strong>rares lymphocytes de taille moyenne \u00e0 grande<\/strong>, caract\u00e9ris\u00e9s par :<\/p>\n<ul>\n<li>un cytoplasme <strong>largement d\u00e9velopp\u00e9<\/strong> et intens\u00e9ment basophile,<\/li>\n<li>parfois un <strong>halo clair p\u00e9rinucl\u00e9aire<\/strong> (appareil de Golgi),<\/li>\n<li>des anomalies nucl\u00e9aires possibles (indentations, formes irr\u00e9guli\u00e8res, nucl\u00e9oles apparents).<\/li>\n<\/ul>\n<p>Ils peuvent \u00eatre observ\u00e9s dans de <strong>nombreuses situations inflammatoires<\/strong>, particuli\u00e8rement chez les animaux <strong>jeunes<\/strong>, fr\u00e9quemment soumis \u00e0 de nouveaux stimuli antig\u00e9niques.<\/p>\n<p>Leur augmentation n\u2019est <strong>pas sp\u00e9cifique<\/strong> d\u2019un \u00e9tat pathologique particulier.<br \/>\nIl est toutefois important de <strong>prendre en compte leur nombre<\/strong> : s\u2019il augmente dans un contexte clinique donn\u00e9, un <strong>d\u00e9sordre lymphoprolif\u00e9ratif<\/strong> ne peut \u00eatre compl\u00e8tement exclu.<\/p>\n<figure id=\"attachment_48585\" aria-describedby=\"caption-attachment-48585\" style=\"width: 768px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" class=\"size-medium_large wp-image-48585\" src=\"https:\/\/www.mylav.net\/wp-content\/uploads\/2026\/01\/bf09-768x454.jpg\" alt=\"\" width=\"768\" height=\"454\" \/><figcaption id=\"caption-attachment-48585\" class=\"wp-caption-text\">Figure 9. Lymphocyte r\u00e9actif.<\/figcaption><\/figure>\n<p>&nbsp;<\/p>\n<p><strong>LYMPHOCYTES GRANULAIRES<\/strong><\/p>\n<p>Pr\u00e9sence de lymphocytes de petite \u00e0 moyenne taille pr\u00e9sentant de <strong>fines granulations ros\u00e9es<\/strong> dans le cytoplasme.<\/p>\n<p>En nombre limit\u00e9, ils ne sont pas sp\u00e9cifiques d\u2019un processus pathologique particulier.<\/p>\n<p>Chez le chien, en cas de <strong>lymphocytose domin\u00e9e par des lymphocytes granulaires<\/strong>, il convient d\u2019envisager une <strong>prolif\u00e9ration de lymphocytes T cytotoxiques<\/strong>, d\u00e9crite notamment lors d\u2019<strong>ehrlichiose chronique<\/strong> ou de <strong>leuc\u00e9mie lymphocytaire chronique<\/strong> (ph\u00e9notype CD8+ CD4\u2212).<\/p>\n<p>Chez le chat, le <strong>lymphome des lymphocytes granulaires<\/strong> (LGL, <em>large granular lymphocyte<\/em>) peut s\u2019accompagner d\u2019une infiltration sanguine ; ces cellules ont alors un <strong>pleiomorphisme marqu\u00e9<\/strong> (taille augment\u00e9e, granulations irr\u00e9guli\u00e8res et volumineuses).<\/p>\n<figure id=\"attachment_48594\" aria-describedby=\"caption-attachment-48594\" style=\"width: 768px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-48594 size-medium_large\" src=\"https:\/\/www.mylav.net\/wp-content\/uploads\/2026\/01\/bf10-768x289.jpg\" alt=\"\" width=\"768\" height=\"289\" \/><figcaption id=\"caption-attachment-48594\" class=\"wp-caption-text\">Figure 10. \u00c0 gauche : frottis sanguin, chien &#8211; nombreux lymphocytes granulaires : leuc\u00e9mie lymphocytaire chronique. \u00c0 droite : frottis sanguin, chat &#8211; lymphocytes granulaires atypiques avec granulations magenta volumineuses : infiltration sanguine d\u2019un lymphome LGL.<\/figcaption><\/figure>\n<p>&nbsp;<\/p>\n<p><strong>CELLULES NON CLASSIFIABLES<\/strong><\/p>\n<p>Il s\u2019agit de cellules circulantes dont la <strong>lign\u00e9e cellulaire d\u2019origine<\/strong> ne peut \u00eatre d\u00e9termin\u00e9e sur la seule base des caract\u00e9ristiques morphologiques.<\/p>\n<p>Leur pr\u00e9sence peut \u00eatre associ\u00e9e \u00e0 des conditions <strong>non n\u00e9oplasiques<\/strong>, telles que :<\/p>\n<ul>\n<li>r\u00e9action lymphocytaire majeure,<\/li>\n<li>h\u00e9matopo\u00ef\u00e8se acc\u00e9l\u00e9r\u00e9e en r\u00e9ponse \u00e0 une forte demande (ex. perte sanguine avec r\u00e9g\u00e9n\u00e9ration importante, inflammation aigu\u00eb s\u00e9v\u00e8re).<\/li>\n<\/ul>\n<p>Elles peuvent \u00e9galement traduire des processus <strong>n\u00e9oplasiques<\/strong>, tels que :<\/p>\n<ul>\n<li>leuc\u00e9mie aigu\u00eb,<\/li>\n<li>infiltration sanguine de lymphome,<\/li>\n<li>effraction (spillover) d\u2019une tumeur solide non lympho\u00efde.<\/li>\n<\/ul>\n<p>Lorsque ces cellules sont <strong>peu nombreuses<\/strong>, une <strong>surveillance h\u00e9matologique<\/strong> peut \u00eatre envisag\u00e9e dans un premier temps.<br \/>\nEn revanche, si leur nombre devient significatif, des examens compl\u00e9mentaires (immunoph\u00e9notypage par cytom\u00e9trie en flux sur sang p\u00e9riph\u00e9rique, cytologie de moelle osseuse, recherche d\u2019atteinte d\u2019organe pour cytologie cibl\u00e9e) peuvent aider \u00e0 <strong>identifier leur origine<\/strong>.<\/p>\n<figure id=\"attachment_48603\" aria-describedby=\"caption-attachment-48603\" style=\"width: 768px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" class=\"size-medium_large wp-image-48603\" src=\"https:\/\/www.mylav.net\/wp-content\/uploads\/2026\/01\/bf11-768x418.jpg\" alt=\"\" width=\"768\" height=\"418\" \/><figcaption id=\"caption-attachment-48603\" class=\"wp-caption-text\">Figure 11. Ces termes ne pr\u00e9cisent pas le stade de maturation, mais sont le plus souvent associ\u00e9s aux <strong>m\u00e9tarubricytes<\/strong>.<br \/>\nEn conditions normales, on peut en observer <strong>rares<\/strong> (\u2248 1 pour 100 cellules nucl\u00e9es) chez le chien et le chat.<\/p>\n<p>On distingue :<\/p>\n<ul>\n<li><strong>Normoblast\u00e9mie appropri\u00e9e<\/strong> :<br \/>\nles \u00e9rythrocytes nucl\u00e9\u00e9s sont pr\u00e9sents avec une <strong>r\u00e9g\u00e9n\u00e9ration concomitante<\/strong> (r\u00e9ticulocytose), en r\u00e9ponse \u00e0 un <strong>\u00e9v\u00e9nement h\u00e9molytique ou h\u00e9morragique aigu<\/strong>.<\/li>\n<li><strong>Normoblast\u00e9mie inappropri\u00e9e<\/strong> :<br \/>\npr\u00e9sence d\u2019\u00e9rythrocytes nucl\u00e9\u00e9s en contexte d\u2019<strong>an\u00e9mie non r\u00e9g\u00e9n\u00e9rative<\/strong> ou <strong>sans an\u00e9mie<\/strong>.<br \/>\nCela refl\u00e8te une <strong>perte de contr\u00f4le<\/strong> du relargage de ces formes immatures par la moelle osseuse ou par d\u2019autres sites h\u00e9matopo\u00ef\u00e9tiques (ex. rate), pouvant survenir en cas :<\/p>\n<ul>\n<li>d\u2019alt\u00e9ration fonctionnelle du parenchyme spl\u00e9nique (ex. n\u00e9oplasies, spl\u00e9nectomie),<\/li>\n<li>d\u2019atteinte m\u00e9dullaire (ex. rupture de la barri\u00e8re h\u00e9mato-m\u00e9dullaire lors de processus inflammatoires aigus graves, endotox\u00e9mie, coup de chaleur, n\u00e9oplasies, chimioth\u00e9rapie, anomalies cong\u00e9nitales).<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>Il faut \u00e9galement rappeler que la majorit\u00e9 des <strong>analyseurs h\u00e9matologiques automatiques<\/strong> ne distinguent pas les \u00e9rythrocytes nucl\u00e9\u00e9s des leucocytes.
