Voici quelques conseils pour la demander et l\u2019interpr\u00e9ter correctement, afin d\u2019\u00e9viter des r\u00e9sultats peu concluants.<\/span><\/p>\n<p><b>Coloration PAL : de quoi parle-t-on ?<\/b><\/p>\n<p><span style=\"font-weight: 400;\">Il s\u2019agit d\u2019une coloration cytochimique simple, r\u00e9alisable en quelques minutes, gr\u00e2ce \u00e0 un colorant sp\u00e9cifique qui met en \u00e9vidence le contenu enzymatique du cytoplasme.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">Les cellules contenant de la PAL apparaissent plus sombres apr\u00e8s coloration, tandis que les autres restent d\u00e9color\u00e9es.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">Ses principaux atouts sont son faible co\u00fbt, sa rapidit\u00e9\u2026 et surtout le fait qu\u2019elle <\/span><b>peut \u00eatre r\u00e9alis\u00e9e directement sur des \u00e9talements d\u00e9j\u00e0 color\u00e9s pour l\u2019examen cytologique standard<\/b><span style=\"font-weight: 400;\">. Cela permet de s\u00e9lectionner au pr\u00e9alable les \u00e9chantillons pertinents pour cette analyse compl\u00e9mentaire.<\/span><\/p>\n<p><b>Quelles cellules sont positives ?<\/b><\/p>\n<p><span style=\"font-weight: 400;\">De nombreuses cellules peuvent contenir de petites quantit\u00e9s de PAL (par exemple certaines cellules \u00e9pith\u00e9liales et les neutrophiles).<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">Parmi les cellules m\u00e9senchymateuses normales, seules les cellules <\/span><b>ost\u00e9oblastiques<\/b><span style=\"font-weight: 400;\">, responsables de la production de matrice osseuse, pr\u00e9sentent <\/span><b>de fortes concentrations de PAL<\/b><span style=\"font-weight: 400;\"> dans leur cytoplasme.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">Cela explique pourquoi :<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Les <\/span><b>ost\u00e9oblastes normaux ou r\u00e9actifs<\/b><span style=\"font-weight: 400;\"> sont toujours positifs.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">En pratique, on utilise souvent un frottis d\u2019archive contenant de tels ost\u00e9oblastes comme <\/span><b>contr\u00f4le positif<\/b><span style=\"font-weight: 400;\"> de la r\u00e9action.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">Mais attention : ce point am\u00e8ne directement \u00e0 la limitation principale de cette technique.<\/span><\/li>\n<\/ul>\n<p><b>La PAL ne distingue pas ost\u00e9oblastes normaux et ost\u00e9oblastes tumoraux<\/b><\/p>\n<p><span style=\"font-weight: 400;\">C\u2019est la cl\u00e9 \u00e0 retenir : la coloration PAL marque toutes les cellules d\u2019origine ost\u00e9oblastique, qu\u2019elles soient normales, hyperplasiques ou n\u00e9oplasiques.<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">En clair, <\/span><b>si vous cherchez \u00e0 trancher entre un os r\u00e9actif et un os tumoral, la PAL seule ne vous donnera pas la r\u00e9ponse<\/b><span style=\"font-weight: 400;\"> : les deux seront positifs.<\/span><\/p>\n<figure id=\"attachment_39669\" aria-describedby=\"caption-attachment-39669\" style=\"width: 768px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" class=\"size-medium_large wp-image-39669\" src=\"https:\/\/www.mylav.net\/wp-content\/uploads\/2025\/08\/blog-fra-13-ago-1-768x271.png\" alt=\"\" width=\"768\" height=\"271\" \/><figcaption id=\"caption-attachment-39669\" class=\"wp-caption-text\">Figure 1. \u00c0 gauche, un ost\u00e9oblaste normal : avec la coloration de routine MGG, il s\u2019agit d\u2019une cellule arrondie, au cytoplasme basophile et au noyau ovale excentr\u00e9. \u00c0 droite, deux ost\u00e9oblastes normaux apr\u00e8s coloration \u00e0 la PAL pr\u00e9sentent de fines granulations noir\u00e2tres dans le cytoplasme, le rendant plus sombre que lors de la premi\u00e8re coloration MGG. Il s\u2019agit d\u2019une r\u00e9action positive normale \u00e0 la PAL.<\/figcaption><\/figure>\n<p><b>Quand et comment l\u2019utiliser ?<\/b><\/p>\n<ol>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>\u00c9tape initiale<\/b><span style=\"font-weight: 400;\"> : toujours r\u00e9aliser en premier lieu la cytologie standard et l\u2019examen radiographique.<\/span>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Si la cytologie \u00e9voque clairement une production osseuse r\u00e9active, inutile d\u2019ajouter la PAL : les cellules seront de toute fa\u00e7on positives.<\/span><\/li>\n<\/ul>\n<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Cas suspect de tumeur m\u00e9senchymateuse osseuse<\/b><span style=\"font-weight: 400;\"> :<\/span>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Si la cytologie montre une population atypique suspecte de tumeur osseuse, la PAL devient pertinente.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Une positivit\u00e9 permettra alors de confirmer l\u2019origine ost\u00e9oblastique des cellules et donc de <\/span><b>conforter le diagnostic d\u2019ost\u00e9osarcome<\/b><span style=\"font-weight: 400;\">.<\/span><\/li>\n<\/ul>\n<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Autres tumeurs m\u00e9senchymateuses de l\u2019os<\/b><span style=\"font-weight: 400;\"> (chondrosarcome, h\u00e9mangiosarcome, fibrosarcome, sarcome histiocytaire) : elles sont <\/span><b>n\u00e9gatives \u00e0 la PAL<\/b><span style=\"font-weight: 400;\">, ce qui aide \u00e0 affiner le diagnostic lorsqu\u2019on combine la coloration cytochimique et la cytologie standard.<\/span><\/li>\n<\/ol>\n<p><b>Exemple pratique<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Ost\u00e9osarcome<\/b><span style=\"font-weight: 400;\"> : cellules n\u00e9oplasiques m\u00e9senchymateuses pr\u00e9sentant de fines granulations sombres dans le cytoplasme apr\u00e8s PAL \u2192 confirmation d\u2019une origine ost\u00e9oblastique.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Condrosarcome<\/b><span style=\"font-weight: 400;\"> : absence de coloration sombre \u2192 \u00e9limine l\u2019ost\u00e9osarcome et oriente vers un autre type tumoral.<\/span><\/li>\n<\/ul>\n<figure id=\"attachment_39677\" aria-describedby=\"caption-attachment-39677\" style=\"width: 768px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" class=\"size-medium_large wp-image-39677\" src=\"https:\/\/www.mylav.net\/wp-content\/uploads\/2025\/08\/blog-fra-13-ago-2-768x257.png\" alt=\"\" width=\"768\" height=\"257\" \/><figcaption id=\"caption-attachment-39677\" class=\"wp-caption-text\">Figure 2. Cela confirme l\u2019origine ost\u00e9oblastique de cette population cellulaire tumorale. \u00c0 gauche, on observe un ost\u00e9oclaste multinucle\u0301\u00e9 normal, inclus dans la prolif\u00e9ration tumorale, qui est quant \u00e0 lui n\u00e9gatif (cytoplasme rest\u00e9 bleu).<\/figcaption><\/figure>\n<p>&nbsp;<\/p>\n<figure id=\"attachment_39685\" aria-describedby=\"caption-attachment-39685\" style=\"width: 768px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" class=\"size-medium_large wp-image-39685\" src=\"https:\/\/www.mylav.net\/wp-content\/uploads\/2025\/08\/blog-fra-13-ago-3-768x513.jpg\" alt=\"\" width=\"768\" height=\"513\" \/><figcaption id=\"caption-attachment-39685\" class=\"wp-caption-text\">Figure 3. Associ\u00e9 \u00e0 l\u2019aspect radiographique et aux observations cytologiques, ce r\u00e9sultat confirme qu\u2019il ne s\u2019agit pas d\u2019un ost\u00e9osarcome, mais d\u2019une autre tumeur m\u00e9senchymateuse.<\/figcaption><\/figure>\n<p><b>Et chez le chat ?<\/b><\/p>\n<p><span style=\"font-weight: 400;\">La technique est valid\u00e9e chez le chien et recommand\u00e9e par MYLAV. Des essais internes montrent qu\u2019elle fonctionne aussi chez le chat, mais la fr\u00e9quence plus faible des tumeurs osseuses f\u00e9lines n\u00e9cessite davantage de cas pour une validation formelle.<\/span><\/p>\n<p><b>En r\u00e9sum\u00e9, pour un usage optimal :<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>\u00c9tape 1<\/b><span style=\"font-weight: 400;\"> &#8211; Suspicion clinique et radiographique de sarcome osseux :<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">r\u00e9aliser d\u2019abord cytologie standard et biopsie histologique (toujours recommand\u00e9e pour confirmer et orienter la prise en charge th\u00e9rapeutique).<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>\u00c9tape 2<\/b><span style=\"font-weight: 400;\"> &#8211; Cytologie non concluante pour une tumeur :<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">la PAL ne permettra pas de trancher.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>\u00c9tape 3<\/b><span style=\"font-weight: 400;\"> &#8211; Cytologie montrant clairement une tumeur m\u00e9senchymateuse :<\/span><span style=\"font-weight: 400;\"><br \/>\n<\/span><span style=\"font-weight: 400;\">la PAL pourra confirmer ou non qu\u2019il s\u2019agit d\u2019un ost\u00e9osarcome.<\/span><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><b>Dr Walter Bertazzolo<\/b><span style=\"font-weight: 400;\">, DMV, EBVS\u00ae European Specialist in Veterinary Clinical Pathology (Dipl.
