Elle peut se pr\u00e9senter sous une forme <strong>effusive<\/strong> ou <strong>non effusive<\/strong> (\u00e9galement appel\u00e9e PIF \u00ab\u00a0s\u00e8che\u00a0\u00bb).<\/p>\n<p>Malheureusement, le diagnostic d\u00e9finitif n\u2019est pas toujours simple. Une confirmation formelle n\u00e9cessite g\u00e9n\u00e9ralement une mise en \u00e9vidence immunohistochimique du virus dans des tissus obtenus par biopsie ou lors d\u2019un examen post-mortem.<\/p>\n<figure id=\"attachment_33344\" aria-describedby=\"caption-attachment-33344\" style=\"width: 454px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-33344 size-full\" src=\"https:\/\/www.mylav.net\/wp-content\/uploads\/2025\/07\/01_blog_fra_31_luglio.jpg\" alt=\"\" width=\"454\" height=\"340\" \/><figcaption id=\"caption-attachment-33344\" class=\"wp-caption-text\">Figure 1 \u2013 Particules virales color\u00e9es en brun par immunohistochimie, dans une biopsie h\u00e9patique de chat atteint de PIF. On observe des l\u00e9sions focales (pyogranulomes) contenant des particules virales positives au sein de macrophages. Ce r\u00e9sultat confirme d\u00e9finitivement le diagnostic de PIF.<\/figcaption><\/figure>\n<p>Ces examens \u00e9tant co\u00fbteux et parfois impossibles \u00e0 r\u00e9aliser en pratique courante, les laboratoires et chercheurs du monde entier cherchent depuis des ann\u00e9es <strong>\u201cle test parfait\u201d<\/strong> permettant un diagnostic d\u00e9finitif <em>ante-mortem<\/em> de la PIF.<\/p>\n<p>Les analyses h\u00e9mato-biochimiques de routine, l\u2019\u00e9lectrophor\u00e8se des prot\u00e9ines s\u00e9riques, les titrages s\u00e9rologiques pour coronavirus, les examens cytologiques d\u2019organes ou de pr\u00e9l\u00e8vements cavitaires, ainsi que certains biomarqueurs (par exemple l\u2019alpha-1 glycoprot\u00e9ine acide &#8211; AGP \u2013 dans le s\u00e9rum et les \u00e9panchements, ou encore l\u2019activit\u00e9 de la LDH dans les fluides corporels en cas de PIF effusive) apportent tous une valeur diagnostique.<br \/>\nCependant, <strong>aucun de ces tests ne permet, \u00e0 lui seul, de confirmer d\u00e9finitivement la pr\u00e9sence de la maladie<\/strong>.<\/p>\n<p>La biologie mol\u00e9culaire, bien qu\u2019elle permette de d\u00e9tecter le virus dans les tissus et les fluides, pr\u00e9sente aussi des limites techniques (par exemple, des faux n\u00e9gatifs peuvent survenir en cas de mauvaise conservation de l\u2019ARN viral, emp\u00eachant son amplification).<\/p>\n<p>Dans les formes effusives, <strong>l\u2019examen du liquide d\u2019\u00e9panchement est un \u00e9l\u00e9ment cl\u00e9<\/strong> du diagnostic. Il s\u2019agit le plus souvent d\u2019exsudats aseptiques riches en prot\u00e9ines, avec des globulines \u00e9lev\u00e9es (et donc un rapport albumine\/globuline bas), de fortes concentrations d\u2019AGP et de LDH, et une cytologie caract\u00e9ristique.<br \/>\nUne PCR positive pour coronavirus sur ce type d\u2019\u00e9chantillon est \u00e0 consid\u00e9rer comme confirmative.<\/p>\n<figure id=\"attachment_33362\" aria-describedby=\"caption-attachment-33362\" style=\"width: 454px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-33362 size-full\" src=\"https:\/\/www.mylav.net\/wp-content\/uploads\/2025\/07\/02_blog_fra_31_luglio.jpg\" alt=\"\" width=\"454\" height=\"340\" \/><figcaption id=\"caption-attachment-33362\" class=\"wp-caption-text\">Figure 2 \u2013 Examen cytologique apr\u00e8s concentration d\u2019un \u00e9panchement chez un chat atteint de PIF : on note un fond prot\u00e9ique granuleux et une population inflammatoire compos\u00e9e de neutrophiles alt\u00e9r\u00e9s, associ\u00e9s \u00e0 quelques cellules mononucl\u00e9\u00e9es.<\/figcaption><\/figure>\n<p>Avec notre ancien analyseur h\u00e9matologique (ADVIA 2120), nous avions d\u00e9j\u00e0 montr\u00e9 l\u2019int\u00e9r\u00eat du <strong>rapport entre l\u2019activit\u00e9 de la LDH et la concentration cellulaire<\/strong> dans le liquide : les chats atteints de PIF pr\u00e9sentent souvent des taux de LDH disproportionnellement \u00e9lev\u00e9s par rapport au nombre de cellules nucl\u00e9\u00e9es (WBC mesur\u00e9es par les analyseurs).<\/p>\n<p>Aujourd\u2019hui, notre laboratoire utilise un analyseur h\u00e9matologique <strong>Sysmex XN<\/strong>, qui effectue la num\u00e9ration cellulaire des fluides gr\u00e2ce \u00e0 une technologie laser (<em>flow cytometry<\/em>) via deux canaux distincts :<\/p>\n<ul>\n<li><strong>WDF<\/strong> (<em>White Blood Cell Differential by Fluorescence<\/em>)<\/li>\n<li><strong>WNR<\/strong> (<em>White Blood Cell Count and Nucleated Red Blood Cell Count<\/em>)<\/li>\n<\/ul>\n<p>Ce syst\u00e8me permet une meilleure classification des leucocytes et le comptage automatique d\u2019\u00e9ventuels pr\u00e9curseurs \u00e9rythro\u00efdes.<\/p>\n<p>En g\u00e9n\u00e9ral, les deux canaux donnent des r\u00e9sultats tr\u00e8s proches dans les \u00e9panchements\u2026 sauf dans les cas o\u00f9 le fluide est riche en prot\u00e9ines\/mucoprot\u00e9ines, comme c\u2019est souvent le cas en PIF.<\/p>\n<p>Le <strong>Delta-TNC<\/strong> est en r\u00e9alit\u00e9 un <strong>rapport entre WDF et WNR<\/strong>. Dans la majorit\u00e9 des situations, il est proche de 1\u2026 mais dans les \u00e9panchements li\u00e9s \u00e0 la PIF, il peut \u00eatre nettement sup\u00e9rieur (voir exemples ci-dessous).<\/p>\n<figure id=\"attachment_33370\" aria-describedby=\"caption-attachment-33370\" style=\"width: 768px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" class=\"size-medium_large wp-image-33370\" src=\"https:\/\/www.mylav.net\/wp-content\/uploads\/2025\/07\/03_blog_fra_31_luglio-768x406.jpg\" alt=\"\" width=\"768\" height=\"406\" \/><figcaption id=\"caption-attachment-33370\" class=\"wp-caption-text\">Figure 3 \u2013 \u00c9panchement septique avec forte concentration leucocytaire : les deux canaux donnent une num\u00e9ration similaire (environ 150 000 cellules nucl\u00e9\u00e9es\/\u00b5L). Le Delta-TNC est proche de 1.<\/figcaption><\/figure>\n<p>&nbsp;<\/p>\n<figure id=\"attachment_33378\" aria-describedby=\"caption-attachment-33378\" style=\"width: 768px\" class=\"wp-caption aligncenter\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-33378 size-medium_large\" src=\"https:\/\/www.mylav.net\/wp-content\/uploads\/2025\/07\/04_blog_fra_31_luglio-768x408.jpg\" alt=\"\" width=\"768\" height=\"408\" \/><figcaption id=\"caption-attachment-33378\" class=\"wp-caption-text\">Figure 4 \u2013 \u00c9panchement li\u00e9 \u00e0 une PIF avec faible concentration leucocytaire : le WDF est nettement sup\u00e9rieur au WNR, entra\u00eenant un Delta-TNC tr\u00e8s \u00e9lev\u00e9 (&gt;5).<\/figcaption><\/figure>\n<p>Sur la base des donn\u00e9es actuellement publi\u00e9es et de notre exp\u00e9rience au laboratoire, nous pouvons r\u00e9sumer ainsi :<\/p>\n<ol>\n<li>Les chats atteints de PIF pr\u00e9sentent presque toujours des valeurs de Delta\u2011TNC sup\u00e9rieures \u00e0 2 ; une valeur basse de Delta\u2011TNC permet donc d\u2019exclure avec une bonne fiabilit\u00e9 la PIF.<\/li>\n<li>Des valeurs sup\u00e9rieures \u00e0 4,9 confirment avec une sp\u00e9cificit\u00e9 \u00e9lev\u00e9e une PIF.<\/li>\n<li>Les chats non atteints de PIF n\u2019ont pas de valeurs sup\u00e9rieures \u00e0 4,9.<\/li>\n<\/ol>\n<p>Ainsi, en se basant sur ces seuils (<em>cut\u2011offs<\/em>), l\u2019examen cytochimique MYLAV du l\u2019\u00e9panchement cavitaire apporte un \u00e9l\u00e9ment suppl\u00e9mentaire permettant d\u2019orienter vers un diagnostic de certitude <strong>in vivo<\/strong> de la PIF.<\/p>\n<p><strong>R\u00e9f\u00e9rence bibliographique :<\/strong><\/p>\n<p>Lopes <em>et al.<\/em> \u2014 <em>Feline Infectious Peritonitis Effusion Index: A Novel Diagnostic Method and Validation of Flow Cytometry-Based Delta Total Nucleated Cells Analysis on the Sysmex XN-1000V.<\/em><br \/>\n<strong>Veterinary Sciences<\/strong>, 2024, 11, 563. <a href=\"https:\/\/doi.org\/10.3390\/vetsci11110563\">https:\/\/doi.org\/10.3390\/vetsci11110563<\/a><\/p>\n<p>&nbsp;<\/p>\n<p><strong>Walter Bertazzolo<\/strong>, DMV, EBVS\u00ae European Specialist in Veterinary Clinical Pathology (Dipl.
