Le diagnostic d\u00e9finitif n\u2019est pas facile \u00ab in vivo \u00bb, car il faut mettre en \u00e9vidence \u00e0 la fois les alt\u00e9rations cliniques et pathologiques classiques de la maladie et la pr\u00e9sence de l\u2019agent causal (coronavirus).<\/p>\n<p>Parmi les prot\u00e9ines de la phase aigu\u00eb, l\u2019alpha-1-glycoprot\u00e9ine acide (AGP) a toujours \u00e9t\u00e9 consid\u00e9r\u00e9e comme la plus utile pour le diagnostic de la PIF, mais depuis quelques ann\u00e9es, la m\u00e9thode utilis\u00e9e pour la mesurer (l\u2019immunodiffusion radiale) n\u2019est plus disponible.<\/p>\n<p>Depuis 2023, il est enfin possible de mesurer, dans notre laboratoire, l\u2019alpha-1-glycoprot\u00e9ine acide (AGP) dans les s\u00e9rums f\u00e9lins, \u00e0 l\u2019aide d\u2019une m\u00e9thode ELISA r\u00e9cemment valid\u00e9e pour l\u2019esp\u00e8ce.<\/p>\n<p>L\u2019alpha-1-glycoprot\u00e9ine acide (AGP) est une prot\u00e9ine positive de la phase aigu\u00eb (PPA). Sa concentration augmente donc au cours du processus inflammatoire. Chez le chat, l\u2019AGP peut \u00eatre mesur\u00e9e par des m\u00e9thodes immunologiques (par ex : ELISA), qui n\u00e9cessitent toutefois des anticorps sp\u00e9cifiques de la prot\u00e9ine f\u00e9line, car l\u2019utilisation d\u2019anticorps d\u00e9velopp\u00e9s chez d\u2019autres esp\u00e8ces d\u00e9tecte d\u2019autres prot\u00e9ines que l\u2019AGP f\u00e9line et donne donc des r\u00e9sultats qui ne correspondent pas \u00e0 la concentration r\u00e9elle de la mol\u00e9cule. Pendant plusieurs ann\u00e9es, il n\u2019a pas \u00e9t\u00e9 possible de d\u00e9terminer avec pr\u00e9cision l\u2019AGP car le kit d\u2019immunodiffusion radiale, qui \u00e9tait consid\u00e9r\u00e9 comme le seul capable de reconna\u00eetre l\u2019AGP f\u00e9line, a \u00e9t\u00e9 abandonn\u00e9. Toutefois, un test ELISA a r\u00e9cemment \u00e9t\u00e9 mis au point avec une pr\u00e9cision et une exactitude suffisante pour une utilisation \u00e0 des fins de diagnostic.<\/p>\n<p>Parmi les prot\u00e9ines de la phase aigu\u00eb, l\u2019AGP se comporte chez presque toutes les esp\u00e8ces comme une PPA mineure ou mod\u00e9r\u00e9e. En effet, ses augmentations sont quantitativement limit\u00e9es : dans le cas d\u2019un processus inflammatoire, des augmentations de 4 \u00e0 5 fois les concentrations normales sont d\u00e9tect\u00e9es (dans le sang d\u2019individus sains, elle est pr\u00e9sente \u00e0 des concentrations inf\u00e9rieures \u00e0 0,5 mg\/mL chez presque toutes les esp\u00e8ces).<\/p>\n<p>M\u00eame chez le chat, dans la plupart des affections inflammatoires l\u00e9g\u00e8res ou non sp\u00e9cifiques, l\u2019AGP pr\u00e9sente des augmentations s\u00e9riques mod\u00e9r\u00e9es. La p\u00e9ritonite infectieuse f\u00e9line (PIF) fait figure d\u2019exception, car les augmentations de la concentration s\u00e9rique d\u2019AGP sont souvent significatives (3 \u00e0 10 fois la concentration normale).<\/p>\n<p>Cela fait de l\u2019AGP un outil de diagnostic tr\u00e8s <strong>important<\/strong> dans les cas de PIF humide ou s\u00e8che, et en fait un <strong>marqueur de choix<\/strong>, parmi les PPA, pour diff\u00e9rencier cette maladie d\u2019autres affections inflammatoires ou infectieuses pr\u00e9sentant une symptomatologie similaire.<\/p>\n<p>En effet, si la concentration d\u2019autres prot\u00e9ines inflammatoires, telles que la s\u00e9rum amylo\u00efde A (SAA) et l\u2019haptoglobine (Hp), augmente \u00e9galement au cours de la PIF, les augmentations de ces derni\u00e8res sont tr\u00e8s peu sp\u00e9cifiques : la SAA en particulier augmente beaucoup (dix ou cent fois les valeurs normales) en cas de PIF mais \u00e9galement avec d\u2019autres maladies inflammatoires, et au contraire, l\u2019Hp enregistre des augmentations  modestes \u00e0 mod\u00e9r\u00e9es, \u00e0 la fois au cours d\u2019une PIF et pendant d\u2019autres maladies.<\/p>\n<p>Les PPA mentionn\u00e9es ci-dessus ne peuvent donc pas \u00eatre utilis\u00e9es pour diff\u00e9rencier la PIF d\u2019autres maladies ayant une pr\u00e9sentation similaire. Bien entendu, m\u00eame au cours de la PIF, on peut occasionnellement observer des augmentations mod\u00e9r\u00e9es de l\u2019AGP, mais qui restent toutefois sup\u00e9rieures aux valeurs normales. Dans ce cas, il est important de corr\u00e9ler les augmentations de l\u2019AGP avec le tableau clinique et toute autre modification biologique compatible avec la PIF ; notamment l\u2019\u00e9lectrophor\u00e8se des prot\u00e9ines s\u00e9riques, qui dans une PIF montre g\u00e9n\u00e9ralement des pics d\u2019alpha 2 et de gamma globulines, et l\u2019analyse cytochimique de l\u2019\u00e9panchement (montrant un taux \u00e9lev\u00e9 de prot\u00e9ines, un ratio albumine\/globuline <1,0, un rapport LDH\/TNCC \u00e9lev\u00e9 et une cytologie inflammatoire non sp\u00e9cifique caract\u00e9ris\u00e9e principalement par des neutrophiles non d\u00e9g\u00e9n\u00e9r\u00e9s et une trame granuleuse) ; il est recommand\u00e9 d\u2019y associer  une PCR du coronavirus. Bien que chez un tiers des chats atteints de PIF, elle puisse \u00eatre n\u00e9gative (sensibilit\u00e9 de la PCR sur l\u2019\u00e9panchement d\u2019environ 70 %), elle devrait confirmer le diagnostic en cas de positivit\u00e9 (sp\u00e9cificit\u00e9 de la PCR sur l\u2019\u00e9panchement proche de 100 %),\n\nEn revanche, des valeurs d\u2019AGP dans les intervalles de r\u00e9f\u00e9rence tendent \u00e0 exclure la PIF et conduisent \u00e0 rechercher d\u2019autres causes pour les sympt\u00f4mes d\u00e9tect\u00e9s. De m\u00eame, il n\u2019est pas utile de mesurer l\u2019AGP chez des chats en bonne sant\u00e9 mais qui ont une s\u00e9rologie positive au coronavirus pour essayer de pr\u00e9dire le d\u00e9veloppement de la PIF.\n\nL\u2019AGP est en fait un marqueur inflammatoire, et m\u00eame si le chat est s\u00e9ropositif, sa concentration s\u00e9rique reste normale jusqu\u2019\u00e0 ce que l\u2019inflammation associ\u00e9e \u00e0 la maladie induite par le coronavirus se d\u00e9veloppe. En d\u2019autres termes, l\u2019augmentation de l\u2019AGP ne se produit qu\u2019apr\u00e8s l\u2019apparition des sympt\u00f4mes et non avant. Pour finir, une \u00e9tude r\u00e9cente men\u00e9e dans des pays o\u00f9 un traitement par analogues de nucl\u00e9osides peut \u00eatre prescrit et administr\u00e9 pour lutter contre la PIF a montr\u00e9 que l\u2019AGP tend \u00e0 se normaliser plus t\u00f4t que les autres param\u00e8tres biologiques chez les chats qui r\u00e9pondent au traitement, alors qu\u2019elle reste inchang\u00e9e chez les chats pour lesquels le traitement ne donne pas les r\u00e9sultats escompt\u00e9s.\n\n<img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/www.mylav.net\/wp-content\/uploads\/2022\/12\/blog_PIF_fr.png\" alt=\"Suspicion clinique et biologique de PIF\" width=\"900\" height=\"588\" class=\"alignnone size-full wp-image-25819\" \/><\/p>\n<p>En fin de compte, la mesure de l\u2019AGP est donc recommand\u00e9e non seulement dans tout processus inflammatoire, o\u00f9 elle montre g\u00e9n\u00e9ralement des augmentations modestes conformes \u00e0 celles d\u2019autres PPA, mais surtout lorsque les sympt\u00f4mes d\u00e9tect\u00e9s conduisent \u00e0 une suspicion clinique de PIF s\u00e8che ou humide : si les valeurs de l\u2019AGP se situent dans les intervalles de r\u00e9f\u00e9rence, la PIF peut \u00eatre exclue ; si, en revanche, les valeurs ont l\u00e9g\u00e8rement augment\u00e9es, la PIF doit \u00eatre consid\u00e9r\u00e9e comme possible si les autres alt\u00e9rations cliniques ou biologiques la confirment ; si les valeurs sont tr\u00e8s \u00e9lev\u00e9es, l\u2019AGP peut \u00eatre un outil de confirmation du diagnostic et, lorsqu\u2019un traitement avec des m\u00e9dicaments antiviraux autoris\u00e9s sera possible, elle pourra \u00eatre consid\u00e9r\u00e9e comme le marqueur id\u00e9al pour surveiller le traitement et pour identifier les \u00ab r\u00e9pondeurs \u00bb.<\/p>\n<div class=\"riferimenti\">\n<h5>Bibliographie :<\/h5>\n<p>Addie DD et al (2022) Alpha-1 Acid Glycoprotein Reduction Differentiated Recovery from Remission in a Small Cohort of Cats Treated for Feline Infectious Peritonitis.
